Just done mine, and tensioners, belts and coolant. To be honest, I probably wouldn't do it myself again. I went for all BMW OEM parts and in total spent about £300 for the lot..

The belt deflector pulley is only on the later engines and is fitted on an extended alternator top support bolt. It improves the belt 'wrap' on the alternator. I just knocked it off with a hammer to loosen it. Same for me. If you're not sure what it is then I'd get that looked at by a specialist. The symptoms you describe could be the signs of impending water pump failure. When the bearings wear they get noisy. Try wobbling the fan from front to back (it's mounted on the water pump shaft) - if there's any play at all...
