I cannot see when replying to a PM any opportunity to add a picture.

Any advice please?

Just treat is as a normal post John, copy and paste the picture links from photobucket, imageshack etc. in to the PM.

If I want to add a pic into a post I am clear on that in there is an Upload Attachment button.

Cannot see the same on the PM screen.

John, the upload button is not normally used, pictures are hosted on other sites (photobucket, imageshack etc.) and the links are pasted in to the posts as and when required. See the link below.
