Top door panel loose

Hi Folks,...

The panel on the driver's door on the inside is come loose. It is the panel where you rest your arm on on the inside with the doors up (i.e. black top panel)

I can't see how it is/was fixed in place, anybody can give me pointers?

The top trim screws are directly underneath, sometimes concealed by the leather door card.

This trim is retained by five screws. The four nearest the front of the car are reasonably easy to see and remove. A mirror is a great help in locating them. The fifth screw at the back (where your shoulder is with the door up) is extremely difficult to locate and fix. This screw is the only one which locates in a slot in the top trim, and if loose will allow the top tim to be removed by pulling inwards.
